Incident investigations

چکیده
This chapter provides principles on how to conduct incident investigations. Incident investigations consist of identifying the underlying causes (sometimes called “root” causes) in a chain events leading an accident, including initiating and failure mitigation, lessons be learnt ways prevent similar accidents future. The investigation should not limited determining immediate or apparent cause(s). Investigations can conducted both by industry public authorities.
